Hexapod frames are an advanced form of external fixator using six adjustable struts and computer software to plan precise corrections. They are particularly useful when a patient needs lengthening combined with angular or rotational correction at the same time. Internal Lengthening Nail (PRECICE) and LON Method

FITBONE Internal Lengthening Nail

PRECICE nails are no longer available in India; FITBONE lengthening nails are used instead. FITBONE is inserted through the knee and uses electrical impulses transmitted to a receiver beneath the skin to activate the internal lengthening mechanism. Unlike PRECICE, which uses a magnetic actuator and was inserted through the hip, FITBONE nails are thicker and stronger. However, both systems are non-weight-bearing during the lengthening phase, requiring patients to use a wheelchair until lengthening is complete and early bone formation is visible on X-rays. LON Method (Lengthening Over Nail)

LON is a hybrid limb-lengthening technique that combines an internal nail with an external fixator. Unlike FITBONE, the internal nail does not perform the lengthening. Once the desired length is achieved, the internal nail is locked to maintain the new length, and the external fixator is typically removed after around 3–4 months. Pre-operative planning starts with full-length standing X-rays, CT scans, and software-assisted planning to calculate the exact correction. On the day of surgery, the bone is cut in a procedure called an osteotomy. The lengthening device is then secured to the bone.

For the first 5 to 7 days after surgery, the device is left in place but not adjusted. This is the latency phase, where the body starts healing before distraction begins. After this, the distraction phase starts. The device is adjusted by approximately 1 mm per day, usually in four small increments spread across the day.
